Tickets can be purchased by completing this Healthy Harvest form and dropping it off at one of our offices, faxing it to 604-851-4898 or emailing it back to email@example.com.
The funds will be withdrawn through Fraser Health payroll deduction.
Proceeds of the Healthy Harvest 50/50 will be used to purchase priority equipment for our hospitals and health care facilities.
Participants must be full-time, part-time or casual, Fraser Health Authority employee
Ticket purchasers must be 19 years of age or older. If a winning ticket bears a minor’s name, the prize will be lawfully delivered on behalf of the minor to the minor’s parent, legal guardian or trustee
FVHCF staff and Board of Directors are not permitted to participate in the lottery.
Draws take place every two weeks on the Monday following payday between 2-4pm at our Abbotsford Regional Hospital office. An email will be sent out to all sites to announce the winner.
Winners name will be announced through weekly hospital email and our Foundation newsletter.
You can sign up for the 50/50 with any time. The Foundation will contact you annually to confirm and renew your enrollment.
Winners do not have to be present at the draw to win.

The lottery is not considered a charitable donation by the Canada Revenue Agency and is therefore not eligible for a tax receipt.
Gaming regulations stipulate all players must reapply annually.

It will take one to two pay periods to remove you from the lottery.
